3 Session Package CC Authorization Multi Payment

I authorize Embodied Bliss to charge my credit card for agreed upon purchases, including 2 recurring monthly payments of $155 for a 3 Session Well Being Package until paid in full ($310) or 3 recurring monthly payments of $105 for a 3 Session Well Being Package until paid in full ($315). I understand that my information will be saved on a secure server for future transactions on my account. I understand that my authorization is subject to my program agreement, terms of purchase, and other company policies, including regarding refunds, of Embodied Bliss.

I have read the foregoing Agreement, understand its terms, and agree.
